The accident - SPOILER!

Can someone please explain to me how Vera's accident actually happened? I can't seem to understand. I saw she got cought up on that hook but what was actually caught? The hook wasn't nearly pointy enough or moved fast enough to got under her skin. If it got caught in her turban it would've just slip off. If it got caught up in her hair... They weren't allowed to use hairpins of anything like that and they showed that they usually just got their hair laid down and pushed it under the turbans. So it's not like the hook could get under a tight knot or something. So how could it happen? One would think that if it just got caught between her locks while they were laid down from her weight it should've slip off, especially because I think she got fairly short hair. Maybe tear off some locks, yes, but a big chunk of her sculp? I can't imagine that.
