It's been a while since I saw it. I can't remember why the warden's assistant (funny fat guy) died. And what about the doctor? And how did the whole thing end? I seem to recall Eric Robert's character ended up on board with the warden's idea of inmates centipedes.

The doctor got exactly what he wanted - a human centipede of all his prison inmates.
He shot his assistant cause he wanted all the fame for himself.
And then Dr. Heiter wakes up and it turns out the entire trilogy was a dream. This however gives him the inspiration to make a centipede out of people so he sets out to find three victims which restarts the chain (no pun intended) of events.
