Also I do not understand the bit where Sarah calls her pretend mum, there is a lady with all these phones with names on which will be the other agents but what was the point of the call?.
She cut herself, from what I understand so it looked like she slipped through the handcuffs and cut herself and so that he would take her to get help because that's when she realized he was in the east.
The phone thing was the fake phone she had with preset numbers, like all the undercovers do from that company in the off chance they are in a situation like that.

I really liked the phone call set up and thought that was extremely cleaver. It showed that they thought out some scenarios in which their agents may come in and prepared for them.
The one I thought was odd was Sarah's need to vomit up items. Yuck.
