I actually had a similar thing happen in an apartment. It was an older building and a nail was sticking up from a floor board, it didn't have much of a head at all. I'd just moved in and was barefoot and it punctured my heel. Fortunately, I was already inoculated for tetanus.

I honestly don't pay attention to the floors in the show, but I recall the episode you're talking about. I'm currently fixing up an old house I just got and I can tell you that depending on the floor type (hardwood) yes it could have a nail. Like another poster said, it's not the usual fat head type. It's more of a finishing nail that barely has a head and is used to help keep the floor boards attached to the subfloor (even though the wood planks are tongue and groove). The idea that they wouldn't either try to get it removed or try to hammer it back in is unrealistic to me.
