First, obviously, the Winter Soldier didn't shoot him to death, but did critically wound him. He explains to Steve and Nat after they see him alive that there was some medicine that Bruce Banner had been working on as a calming agent... Didn't work very well for Banner but Fury was clearly able to make use of it. It appears to slow bodily functions down to a point where it mimics death (but doesn't actually cause it). The head doctor that was working on him when he was brought into the ER is the same one overseeing his care at the hidden facility (who is played by one of the directors), so he was 'in on it'

Pay attention to when Bucky is getting repaired after the causeway fight and is having flashbacks. He survived the fall (though lost his left arm in the process) and was found by Russian agents. The writers of the film have indicated that these are meant to be agents from a Russian cell of HYDRA.

As for how he survived the fall in the first place. Remember the first movie when Steve rescued Bucky from that HYDRA facility, Bucky was being experimented on (Steve even notes in this film that whatever they'd done to Bucky back in '43 mist have allowed him to survive the fall). Outside information (interviews with cast/crew) indicates the idea is that Bucky did in fact receive a version of the super soldier serum (likely Zola's attempt at it after studying the Red Skull's blood... Which could explain why he'd gotten closer to replicating what Erskine did than anyone else for quite some time). Now why he didn't seem changed at all right after Steve rescued him in '43? My own personal theories are that, because this wasn't Erskine's exact formula, it may have laid dormant in his system until the trauma of the fall 'kicked it into gear' and/or that what was in his system at the time was just enough to keep him alive after the fall and it was only through subsequent/continued experimentation that he became more equivalent to Cap (in strength/agility)

Again, those are just my own personal theories

Agent Hill: ...Then aliens invaded New York and were beaten back, by among others, a giant green monster, a costumed hero from the 40's.... and a god.

Agent Ward: I don't think Thor is technically a god.

Hill: Well...you haven't been near his arms.

He didn't age because they keep him in hibernation between one mission and the other.

All the idea of the winter soldier came from the Cold war paranoia of the Soviets having sleeper agents who could be activated through vocal commands, there is an excellent movie with Charles Bronson on that.
