SPOILER ALERT!!!

What was the whole heart transplant thing? Was it real? Did Emily real get Victoria's heart or was it just a nightmare?
It was really creepy! Seems unlikely to be true and as a nightmare, I didn't get the idea behind it??

The writers said they intentionally left it up to viewers imagination if it happened or not.

I think it's unlikely to be real (and also somewhat creepy) and as a dream it's just weird.

Emily really did get a heart transplant, probably after Victoria shot her. You can see the scar on her chest when she's getting ready for her wedding. However, its suggested that the heart being Victoria's was just a nightmare. I read an interview with Madeleine Stowe (Victoria) and she said that the original script had that really happening, but they changed it to let the audience make what they wanted of it and to be more "poetic".
