No. If Clay filled it out on his own, it probably would have, but Jeff took it and changed the answers to try and help him.

That's what I was thinking, too. I wonder whom Clay got, though... He was very shy and secretive about the whole thing, so I thought that line would be continued but it wasn't.

I looked at it again and confirmed, she was the first on his list:

I guess it was obvious, since he still had it in his pocket. He was just waiting to see if she said he was on her list.

This is one of the "missed opportunities" that I found the most troubling. Why wouldn't he have told her?! Very strange. He could have at least brought it up jokingly, like, "Guess who my number 1 match was?!" Clay was such a spineless pussy.
