Supposedly it was Madonna herself who wanted him to parody "Like A Virgin" as 'Like A Surgeon". The story goes that Madonna & her publicist were watching his "Eat It" video & she wondered when Al would parody her & actually came up with the title. The publicist happened to know Al's agent or publicist & got the suggestion to him.

Yep. It was the Jerry Falwell vs. Larry Flynt libel case over the
Hustler magazine Campri ad parody that went all the way to SCOTUS that set the precedent on that.
