It seems like it would be, but on the other hand you don't know the context of the image or what leads up to it or even exactly what is going on in it until it happens. I think it's not too bad.

In fact, I thought the poster was really misleading.
Before the movie, the poster made me think that I would be watching two girls taking revenge and fighting back.
I was quite surprised to the relationship they had and what the events had lead up to between them!

However, the alternate title of the movie on Wikipedia, "The Whole Story of the Kim Bok-nam Murder Case", is insanely spoilery and I almost didn't watch the movie once I saw it. :/
The alternate title is just insane. Seriously, I watched this movie without paying attention to the poster, I got absolutely zero spoilers, so I had NO idea what was coming. I watched the movie's trailer on youtube after the fact. That trailer would have almost completely ruined this movie for me.
Thanks to not getting the movie spoiled, this ended up being one of the best movies I've ever seen. I think the key is not knowing before-hand that Bok-nam was going to go insane.
