at the end frank is revealed to be a little disfigured, he has scars going all the way around his head and he has patch balding. we're these caused by the helmet/mask itself ? that is what I thought

Yep, from the head. You'd figure if he started wearing heads when he was fourteen, twenty years or so of head wearing would be the cause.
