The scene was simply shot to showcase the anger inside of Julian. It don't matter whether the dude lived or died.

You can still close your mouth back up. In fact thats likely to be exactly what someone's reflex or auto defense mechanism would do. Not go limp and let it happen. With pain you tense up. Thats why you always see people putting things in peoples mouths during painful on the fly surgery so they can bite down on them.
