I've not seen the preview yet (haven't found one viewable outside of Australia: I'm in Britain and dear god is the BBC Angry Boys website crappy. At least the HBO/ABC website have had a little effort put into them), but as far as I could make out from the HBO website, I think the condition just worsened over time, of its own accord. He only had the operation in one ear, right? I guess it just didn't work out so well, it was probably likely that the operation wouldn't work perfectly, but they went for it anyway, just on the hope of it. It's sad, but it's a great idea, and I was so glad to see he's bringing them back, especially since I was just expecting Ja'mie again ( I think she's been exhausted now). I think it just shows how much thought and care he puts into the characters, rather than just using them for humour. He's a brilliant writer, and I can't wait to see who else he comes up with/brings back.
EDIT: I take back what I said about the BBC website, I just checked it out and they've updated with pictures of all of the new characters, though no descriptions, and no trailer as of yet.
EDIT EDIT: As of this morning, the link to the BBC website has been taken down from the main Angry Boys site, and the pictures of all of the characters have been removed from the BBC site. I think the original plan was that they would be revealing two new characters with each new trailer over the coming weeks, but the BBC screwed that up. Someone must be p*ssed. If the BBC get punished with a later airdate for this, I'm gonna be so annoyed.

They got taken down, and the link to the BBC website is gone from angryboys.com. Like I said, I figure that their plan had been to slowly reveal the characters over the next few weeks, and the BBC screwed that up by posting all of them up there, most likely with no permission. So as a result, they seem to have been taken off of the main website. I'm really hoping this doesn't extend to the British airing of the show.
Anyway, I don't have the pictures, but to the best of my recollection, the other three characters were (it was name and picture only... Nan, which was basically just Chris Lilley as an old lady, and two other guys who's names I can't remember. One was that surfer guy that's been seen in a few publicity photos already, and the other was a kind of pale-ish guy wearing a big parka coat. My first thought was that he was some kind of musician. Like a douchebag rapper kind of thing But aside from that, there was no other returning characters. Just Daniel and Nathan.
