Who was the first kidnapped Carrie-Anne girl who got her neck broken?

Four quick questions -
1. Who was the first Carrie-Anne who escaped and ran towards the car with the couple in it who were about to make out?
2. How and why did the second Carrie-Anne (the waitress from the diner) who attacked J-Law die?
3. For such a small town how come nobody raised any suspicion over girls going missing (assuming this is what he's been doing for a while, kidnapping and turning girls into his dead sister)?
4. Why were these Carrie-Anne's so aggressive? The first tried to attack the couple in the car and the second attacked J-Law. The only people they didnt seem to attack was their kidnapper himself. Was it fear and panic and confusion that made them act that way?

Some arbitrary female

Over does of chloroform or some other drug.

Good question

Who is J-Law? She didn't attack the couple, just gave them a scare.

1. It was presumably the initial girl that Ryan kidnapped and delusionally hid in his house as "Carrie Anne."
2. Elissa was to be the new "Carrie Anne" and Ryan didn't want two so he killed the Penn-State "Carrie Anne." We don't actually see him kill her, but her dead body was placed in the trunk of Ryan's car for later disposal.
3. Because there was -- evidently -- only one girl missing, which would've occurred up to four years earlier. Ryan's kidnapping of Peggy, the waitress, was too recent to be known; in fact, the police probably weren't even notified yet since a person would have to be missing at least 48 hours before the police can investigate.
4. Ryan injected them with a drug to make them deranged in order to fit his delusion about Carrie Anne being mentally disturbed and life-threatening. It's unclear, though, how life-threatening either "Carrie-Anne" really was. For instance, the "Carrie-Anne" that escapes and goes to the car where the youths were making out, was she going to try to kill them or was she seeking help to escape Ryan? I think the latter. If so, the knife she had would've been for her protection.
