Oh, Kathleen. When a patient tells you that the world would be better without them in it, you don't pause and ask stupid questions. If Catelynn expressed suicidal thoughts so soon after leaving rehab, you should commit them!
Catelynn is beyond depressed and is not functioning at all. Commit her!
I didn't think it was a stupid question. I think she was really trying to get cate to really think about it & then realize what she was thinking was not true at all. Like she really wanted her to think about how the people in her life would be broken without her. That's why she told her that & asked her "would it?"

Hospitalization only occurs when there is active suicidal ideation, not just passive ideation. Most hospitals won't even hold someone for passive ideation. If they are not in imminent danger, they don't get held. For hospitalization to happen, Cate would need to express a plan, means, and intent. That was not present from what we saw, but what was bothersome was that the therapist did not inquire further and ask those important questions to see just how deeply Cate was thinking in that manner. It's not unusual for a client to say a comment like "the world is better off without me" and when you probe further they reveal a plan, means to do it, and timeframe. Maybe she did ask and it was not shown?
While the questions were not horrible per se and were meaning to lead Cate to think more deeply about her comment and see the lack of logic behind it and her significance, she definitely should not have just glossed over the comment. For me, anytime a client reveals suicidal ideation, even the most passive, we discuss it deeply and develop a No Harm Contract/Safety Plan. Cate's spiral is definitely alarming, so I would hope her therapist would be seeing this and would be keeping an eye for at what point safety becomes a concern. There are already so many red flags.
