When he finally cut off his arm, why was he not utterly screaming in pain?

He was screaming during the cutting, but he didn't scream after -- not even a shout of pain.

Shock,relief,adrenaline.

Definite shock victims with lower than normal blood volume just don't have energy to complain of pain. It is the fight or flight response along with adrenaline keeping him going. Plus he probably had a rush of relief even after the detachment.
