I've personally seen enough fake breasts (in movies and TV shows) to note the difference between them and the real thing and you can tell from the shape of Dunst's that they're real. Also, if they were fake, when she was lying down naked partway through the movie they'd be stick up but they were slightly spread out and how they moved. Yes that's what natural breasts look like (gravity pulling down) as opposed to fake breasts being un-naturally spherical.
