The minute I learned about Annie's heart condition, the only thing I could think of is that they are 'positioning' the show to bring it to an end.
Do we already know that this is the last season (which would make me the last-one-to-the-party)?
Hope is a good thing, maybe the best of things, and no good thing ever dies.

It's not that uncommon that a "health risk" is introduced as a season long story arc only to be resolved by season's end.
I also think they just needed something new to spice up the storyline. I mean prior to last season, the series was pretty episodic, with a subtle ongoing arc in the background and Annie's Spy-a-week antics being the center of attention. Last season changed where it's pretty much one giant arc for the entire season...and since last season's arc...more or less wrapped up the last 3 seasons ongoing arcs...looks like they ran out of things to do so why not throw in a heart condition

Didn't Annie tell Eyal it's fixable with a heart valve replacement operation?
She then said there were a lot of risks with that too, so she'd opted to continue on without treatment.
But then we saw through that logic later, when she admitted she just wanted to stay in the field--and that's why no operation. She'd obviously have to tell Langley if she did the heart valve replacement and was off for that long.
The way these shows work is, if it's renewed, we'll prolly see her collapse in the last scenes of the season when in a meeting at Langley, ergo things will be taken out of her hands when the doctors do emergency heart valve replacement surgery while she's unconscious.
The last scene will be Annie on a gurney being wheeled into surgery. ha
Then that will be the cliff hanger for season 6. What happens after the surgery? Back in the field? Not? Full recovery? Not?

That's my line of thinking, as well.
If they find out about renewal before they've put the finishing touches on the last few scripts, then they can really make it a nice ending with closure either way.
There are so many options.
But folks on the board already know my favorite---she shows up at Eyal's house or on his boat after finding out she's not healthy enough to continue being a field agent, and that's that. After all, who else loves her enough and has the patience to put up with her? haha
I know next to nothing about the inside baseball ratings things, but have read that if a show doesn't earn a .40 average in the important demo over all the aired episodes, then it's almost certain to be cancelled. And this series so far as been performing on the edge of that.
Even if the ending is scripted (and/or filmed, a bit) before they find out if they get a new season, things can still change.
For example, last year, the end of S4 had Annie quitting her job and patching up things with Auggie. Then they found out they got a fifth season, so they decided to drag things for one more year.
With that in mind, I do think that if CA gets a S6 (which, unfortunately, I can't really see it happening - more like a few eps to wrap up storylines), the ending will have a cliffhanger of sorts.
