After the hospital shooting, when the cop (the one who was behind the whole thing) walks in front of the Driver and kneels down, why didn't he shoot him? Did the cop not show his face at the bank robbery incident 10 years earlier?
No, Cop only came in at the end and from behind Driver, so Driver never saw his face. This necessitated Cop hiring Killer, because given his age and dependecy issues, plus the fact that he thought he would be recognized and hence couldn't sneak up on Driver, made it unlikely he could kill Driver before being killed himself.
The potential plot hole is that the guy who put the hit list together found everyone except the mastermind. Highly unlikely, but obviously necessary to allow the twist of it being Cop.

How is that a potential plot hole at all? Or even highly unlikely? Shouldn't the criminal mastermind simply be better at covering his tracks?
Hence the fact that he was a LONG-SERVING COP, a cold-blooded murderer AND a high-functioning drug addict.
