shareLoL, good question. Realistically, there is no way Elena would have been a doctor (season 1-3 Elena possibly), but season 5 on Elena was headed towards trophy wife who sells jewelry and develops a drinking problem. This girl did not have the smarts or the motivation to become a doctor.
I see Caroline as a news woman or owning her own wedding planning/party planning company.
I can see Bonnie being a teacher or researcher as someone else said.

I think you're spot on, except I think Damon would have had a good military career. He's willing to take the risks and the heat that falls on him when he saves everyone's backside.
Stefan would be the crazy jack-of-all traded uncle that drifts any way the wind blows and makes all the family gatherings more fun, but never settles down.
Enzo would be a soap opera star. Katherine, I agree would have been a movie star diva much like Liz Taylor with even more broken hearted ex-husbands.
