Jeffrey Dean Morgan's character didn't die. Worthington called the phone to see who it was. He told Lucy the mother to ask what happened to the daughter. Ann didn't actually die (they saved her). But Ann's brother assumed she was dead so she told drug addicted mom. Mom snapped and stabbed glasses guy. Gunfight ensues. Worthington lets glasses guy die a slow and painful death because he thinks his partner died and I guess as a payback for all the young girls they raped and killed.

Ann and Worthington's character drive back to the post-crime scene house to collect whatever items of hers remain. She didn't have much in that house and that house is a terrible memory for her so I assume she didn't want anything from there. Morgan's character ends up assuming guardianship or adopting her and that's where Worthington's character drops her off at the end. Her new home.
