How Does Joe's Cancer Friend Collect Debts

We saw Joe have someone welsh on a 5K bet, which I'm sure happens with gamblers. How does Joe's friend collect? Did they ever say? It seems you would have to have some force behind your operation as a bookie.

it's so weird seeing "little Gianni" as some imposing threat.. seems like only yesterday he was in Ray/Joe's kitchen mismeasuring the area to install a new oven..
