How did Mor'du end up as a bear? Did he go to the witch and ask for something similar by receiving a cake like Merida did?

If you have the DVD then look up the "Ballad of Mor`du" short film in the extras.
There are very strong parallels in the stories which are not coincidental.

I think it was implied during the Witch's conversation with Merida. Also, there's an animated short that explains the history of Mordu.
