The relationship was that of a mother and child who are unable or unwilling to cut the umbilical cord, so to speak. Do-Joon's handicap made him still dependent on his mother, and when he talked about sleeping with her he meant it with the innocent literalness that a child would attach to the word "sleep". Touching his mother's breast was, I think, a reference to breastfeeding.
