I know the ending is intentionally ambiguous, but what are your thoughts?
The way I see it is this: Randy doesn't die, but the day is near. Very near.

I have always thought that Randy Robinson lives. However, that was his last match so his character Randy "The Ram" Robinson died.
Whether he lives or dies doesn't matter.
He didn't care if he lived or died. He cared if he was loved. In his mind the only people that loved him were the fans. So to him nothing else - dying - didn't matter. What mattered was that the fans cheered him.

I think he knew that he had about a 50/50 chance of dying or going to hospital only to die shortly after. But he continued cos he just didn't care, since he only lived to wrestle. I just assumed he died in the ring.

Yeah, I think he may have reconsidered and ended the match had Cassidy/Pam still been there when he looked up (especially since he knew his heart was acting up)...... but when he saw he was alone with just the audience, he said screw it. This is the way he wanted to go out. The look on his face during the last scene was pretty "final". He knew he was going to die then and there. I know they didn't show it, but I feel it was pretty strongly implied. Such a sad movie. I wish he would have given Cassidy a chance to prove she was "really here now", as she said. :(
