Though I find it obnoxious when people include Heather Graham as Casey, and Trudy and Sherie, and Chloe. They weren't real characters, they were Stab characters.
Also if we're going to include unseen deaths, I feel like Maureen Prescott should be included; we've heard enough about it, and hers kicked off the whole thing.
