S10E01 very confusing

Spoiler
I didn't quite understand the whole Amy's pregnancy with twins and the Spartan dream. Then the ground hog effect repeat of trip to the doctor but same results? So was that supposed to be the horse's ultrasound? And I thought when Cass had the pregnancy scare that Amy and Ty didn't want kids right now? Are they just trying to stuff a baby in there to finish off the series with a happy ending for all? There are more zigzags in this, I miss the old days of Amy helping out horses. Georgie is starting to get on my nerves and I'm bored with her tween antics.

Okay, let's try to clear things up.

Amy is pregnant (something we found out about at the end of season 8). She's been having nightmares about Spartan leaving her. I attribute this to the fact she's been advised by her doctor not to ride during her pregnancy. She feels like she's losing a part of herself, and it's manifesting in the form of Spartan abandoning her.

The ultrasound DVD.

We see Amy and Ty driving back from picking up this DVD. We are initially led to believe it's Amy's ultrasound. But think about it: what doctor would give Amy an ultrasound of her baby without fully explaining what was on it?

When she asks whether she's seeing things correctly, of course we're in freak-out mode, because they see "two" of something...

At this time, we do not know they're actually talking about Minnie, the horse's pregnancy. But...

If we pay attention, we'll understand that Ty quickly realised it wasn't a "hay belly" Minnie was carrying. As such, he took her to have an ultrasound. Amy accompanied him. The time jump reveals as much.

Later, Amy goes for her own 12-week ultrasound that was alluded to in the episode, and no, of course she isn't the one pregnant with twins.

Finally, what makes you think Amy isn't working with horses anymore? Keep watching. You might be surprised.

It was basically, "here, this is what happened before so watch it and once we're done with it, we'll go back to the present."

As for ending the show.. one would say that it might stick around for 10 more years depending on its success because this is no normal series.
