Nothing leading up to Richard Gere's heart attack suggests he's going to have a heart attack (other than the dog acting weird before he leaves that day). There should have been some indication earlier in the movie that Gere might have some heart issues.

Sometimes people who live a healthy life just keel over and die. Eating healthy, exercising, not leading an overly stressful life, these things can reduce the risk of sudden death but it doesn't eradicate the risk completely.
So true what hurricane said.. sometimes it just happens. My uncle died suddenly of a heart attack and no one had any idea it was going to happen, including his doctor.
