I noticed that after Dicky and Charlene settled things, Mickey was talking about getting O'Keefe back but I don't think I ever really saw him the rest of the movie. He wasn't apparent in the training and during the title fight it didn't seem like he was there in the corner. Did anyone notice him?

I'm pretty sure he did come back, gotta remember that O'Keefe was also a local cop. Probably couldn't get a week off to fly to England for the fight.
EDIT: Was rewatching this the other day and O'Keefe is at the title fight. There is a quick shot of him getting off the apron when Dickie is talking to Micky. There are some better shots though during the celebration, including Micky hugging him
O'Keefe plays himself in the movie, so I'm guessing he really came back. Otherwise, he would not be still close with the family and asked to be in the film.
