I know it was never really explained why nathan has medicine, but i was wondering if anyone had any theories as to why he has the medicine in the first place?

Depends on which medicine you're referring to?
The medicine he had in the past, which Rotti switched with poison to kill Nathan's wife, Nathan discovered it. He was a doctor before so he had been trying to find a cure when she first got sick. He thought he had it but Rotti switched it. Its briefly mentioned in one of Rotti's songs.
The medicine he gives to Shilo isnt medicine, its Zydrate (my theory). Its briefly mentioned in the song on the stage at the end. He'd been drugging Shilo with it to keep her addicted and whenever she went into withdrawal (the scenes when she blacked out) he would give her more. It was his way of convincing her she was sick and needed him.
Its also why at the end Rotti tells her she just needs to let the symptoms pass. She blacks out without more of the drug but wakes up, still alive. She was never really sick. It was all Nathan's attempt to keep her.
I was referring the medicine for shilo, sorry that was a vague haha. But the zydrate thing would make a lot of sense.

that was my take on it. rotti says something in the end about nathan keeping her sick, poisoning her and even nathan admits it before they tie him to the wheel chair. he tells shilo that he's been making her sick.

He tells her he's trying to find a "cure" for her "disease". So he's been making his own to keep her with him.
I semi-agree with the Zydrate theories...but...

I just figured they were like gelcaps. You can buy empty capsules and put whatever you want in them.

Hmm. I like that idea. We just know Nate created the meds and poisoned Shilo. The Zydrate theory does work. The capsules are small enough to when Shilo takes them, there would only be enough Zydrate to keep Shilo relying on the meds but not enough to give her the 'trip' all the junkies and surgery patients experience. And if she missed a regularly scheduled pill, she'd go in withdrawal and pass out.

I always thought that Nathan was giving Shilo a smaller dose of the poision that he mistakenly gave to Marni to make her sick and dependent on him in a crazy 'Munchausen by proxy' set up.
The Zydrate thing could work as well as we never really see what form the legal zydrate comes in (which Nathan would probably have access too being not only a Repo Man but a doctor as well), we only see illegal version of it.
