None of this is shown to us. The fact that his dead guinea pig was stitched up is not enough to be sure.
This kind of uncertainty reminded me of "Alice, sweet Alice" (AKA Communion); a similarly brilliant psychological thriller.
We also never see Joshua tamper with his mothers medicine. I don't think it was necessary for him to do that.
shareI think it was necessary because he wanted his mom to have a break down and leave so he could live with his uncle. That's the final scene of the movie. Psycho-boy plotted it all along. This kid was great at playing a creep. He made Damien look like an evangelist.
