Explain the ending for me please

Did he get reunited with Anna? Did he die or ???

The only timeline where he ends up with Anna is the one where he waits by the lighthouse every day. We are shown this at the end. This is why 9-year old Nemo chooses neither mom or dad and instead goes to the pier to throw rocks with young Anna.
