Was it all in Ofelia's head? When the general's walking up behind her he just sees her talking to nothingness. What gives?
If the spell was finally broken and she no longer was a mere mortal child, then how come she dies from the gunshot? She just stands there and doesn't move, so I take it she did die, right? What gives?
The king said she succeeded by not drawing an innocent person's blood, but wasn't she herself innocent? Kinda weird, huh?
As for the first question it is one of the most common among people who have watched this amazing (in my opinion) movie. Guillermo del Toro said that he himself believes it was real and not just her imagination. In the movie's commentary he stated that the most obvious clue is that, when Ofelia is locked in the bedroom which is guarded by some men, she escapes the room using only the magical piece of chalk. However, there are others who believe (including me) that it was all in her imagination. But I guess that what really matters is what the director had in mind right? Not really, it is us who must decide. But it doesn't matter if it was real or just her imagination. The point is that nothing that happened in the Fantastical World, no matter how scary and violent, could not compare to the real villain, Vidal.
Ofelia obviously died at the end. We can see the blood and she clearly stops breathing. I can't understand what "spell" you are referring to. I don't remember a spell but I haven't watched the movie for quite some time now.
And finally, yes she was innocent. But she disobeyed the Faun and refused to give him her brother. She knew the Faun would harm him. This was the last test, the most important one. Se sacrificed herself in order to protect the innocent baby, a quality that most rulers should have: altruism.
It is also important to see that the main theme of this movie, as Guillermo himself pointed out, is disobedience and the power to do what you want. Notice how Ofelia didn't open the little door the fairies were pointing to in the Pale Man's room, how she ate the grape even though the Faun specifically told her not to eat anything from that table and how she didn't give him the baby when he asked her to.

By "spell", I meant Ofelia completing all the tasks and returning to the underground kingdom. It was stated at the beginning of the film that once the Princess returns, she no longer remains mortal.
Oh, OK I see. I didn't exactly remember that... (shame on me) But she became immortal at the end. She died in the real world but she continued her life in the imaginary world where she ruled for many centuries. And it is also said that "she left behind small traces of her time on Earth", another type of immortality: the things you leave behind and endure even after you die. For example Mozart became immortal because of his Music - there are many people who know his name. You and I will be remembered by those who love us for the acts of passion we have shown - we are immortal as long as there is someone to remember us.
