Elements in the source novel were based on author Philip K. Dick's experiences in the 1970s drug culture; after his wife left him, his house became a sort of commune for young people who used the drugs of the day. He became addicted to amphetamines, and experienced or witnessed the behaviors depicted in the novel and film. He The list of credits you cite were Dick's, from the end of the novel, and he asserted they were real.

Those are the people that the author of the novel, Philip K. Dick, knew along this life. They were all drug users and he put their name together with the consequences of their behavior. I think it was his way to honor their friendship.

Also, the entry Phil - Pancreatic Damage is referring to Phillip K. Dick himself.
