Like a mysteron double, version from another universe or a reverse flash like character from the future who has been pretending to be the real Dan Scott who died without knowing he got Karen pregnant.

or better yet what if Dan was really Nick Wolfe and was immortal and couldn't be killed and his death in the hospital was a ruse??
