How Were They so Sure the Scar was from an Abortion?

Can't a scar in the vagina just be caused from an injury? She was out in the woods for weeks, so she could had told them that she just fell on some rocks or something.

It's just something the movie doesn't make clear.

A hysterectomy scar (Aviva is alluded to have) is like an elongated c-section scar.
But the doctor was pretty angry about the photos that he took, saying rather strong words etc. So he obviously put two and two together.
