Christian is down in Florida doing transgender surgeries for men and women, still chasing the women despite now being 50 and trying to come to grips with his own mortality and increasing erectile dysfunction. Sean has grown tired of working on orphans in eastern Europe because while the results are satisfying there is no pay and he's missing the money and being away from his kids for long periods of time. Matt has decided to get a sex change to become a woman and Christian is chosen to perform the surgeries. Ava is still a pain in the ass and keeps trying to get into Matt's life. Julia still lives in London and keeps in touch with Christian and Sean.

Good one! Eva goes back to being a man. Julia comes to terms with not having a career as a doctor and comes to appreciate the joy of raising kids and is facing an empty nest. At 50 she wants to have another baby. Sean is no longer fertile and long since had a vasectomy, so she obviously turns to Christian to be the father and Christian is elated to finally raise his own child. Custody issues ensue and The three of them end up living together.
