I've not read the book the film is based on, but from the reaction of Max's mother, I'd say Stella had taken another lover. Or least, behaved in some way mom considered scandalous.

I also watched the movie "Asylum" yesterday and I think she had cheated on her husband from what her son, Charlie, said in the car to his grandmother, that he thought one of the patients had just been in his mother's room. Why would he have assumed that, especially since he liked Edgar, if he hadn't been through that before? Also his coolness towards his mother when she returns briefly to the family in Wales seemed to suggest that she had abandoned him before.

When charlie said to his grandmother, that he thought one of the patients had just been in his mother's room, That was when Edgar was with his with his mother and the grandmother came back to the house because she left her wallett, and Charlie saw Edgar.

That's really interesting, I hadn't considered that before. I haven't read the book, but I saw the film yesterday and I initially thought Charlie said Edgar had been in his mother's room just because he saw Edgar sneak out of the house with his clothes all askew. But it is interesting to consider that maybe he suspected adultery because he had noticed something of the sort happening before. Also, the way Max and Stella were so distant might contribute to that theory (though I suppose they could just have drifted apart): maybe she had had an affair previously, or he suspected her of having one. I read one comment on this board that said that in the book Max married Stella when she was very young, so perhaps even if she hadn't had an affair he was insecure that she might do so since she had been so young when they got married?

Is the book the same as the movie or is there alot more in it ?

The book and the movie are fairly similar. The book is written by Ian Mckellen's character. The major differences are:

SPOILERS IF YOU PLAN ON READING THE BOOK

1. Stella does sleep with Edgar's friend when she is in London.
2. When Stella returns to her husband Edgar is sent back to the Asylum, they never see each other again, he doesn't follow her to Wales like in the movie.
3. I don't remember any mention in the book about Stella having a prior affair.
4. Stella kills herself in the book, but she does it by hording the medication they give her and then overdosing on it.
