If Nicola hadn't insisted that Giorgia go and buy the 3 ice-creams on her own, a very different history might have then followed.
It is interesting how the smallest, seemingly insignificant, decisions in life sometimes end up impacting the wider journey in ways we can never imagine...

I think you've hit the nail dead-center here. Every decision we make in life - no matter how seemingly trivial - could (and usually does) ultimately alter what the future may hold for us, while also affecting many of the people who are, or will become an integral part of our existence. I believe this was a key message the film intended to convey.
Through the years, most of the characters' lives were molded and/or changed from a single decision, by either choice or chance. To name just a few, Nicola insisting that Giorgia buy the ice cream, then choosing the field of psychiatry, which unintentionally led to finding and rescuing her. Giulia abruptly leaving for her cause, only to later warn Francesca of her husband's danger. Sara's whimsical decision to finally forgive her mother.
And in the end, it was a simple glance at a wall poster for a photo exhibit that, from Mirella's "chance" encounter with Matteo, eventually culminated in a renewed spirit and peace for his mother and Mirella through Andrea. This also created a snowball effect which finally - with the help of each other - brought Giorgia out of her shell and a long-lost, much-deserved happiness to Nicola.
However you look at it, an exceptional film that I watched in a single sitting and would do so again.
