Selby was trying to remove it herself, but it made me wonder why they didn't simply go to a doctor because my cousin's wife has never worked, she's had a broken arm and other ailments and she just goes to the public hospital and says she is sick and wants to be treated for free. As far as I know she doesn't have Medicaid.
shareHospital services may have been more complicated for Selby back in 2001-2002 (around the time when these real events took place). I don't know if the real Selby had a broken arm in a cast, but I think that scene did a great job showing their growing desperation, and Selby realizing that Aileen isn't going to be getting a job any time soon.
You may be right and some hospitals may have helped take that cast off for free. But it's probably a "Your Mileage May Vary" situation, and she didn't have the resources or even a car to go find out which hospitals did.
The events took place in 1989-1991. The movie was filmed in 2003.
