Pay close attention to the first 30 seconds of the film, BEFORE the credits roll and LISTEN to what Marie says.

As for the decapitated head fellatio scene, it is symbolizing/foreshadowing the theme of beastly, deadly love (Marie's love for Alex). Marie killed to have Alex all to herself because she was so insanely in love with her. Notice how the decapitated head closely resembles Alex. In other words, love = fellatio, insanity/murder = severed head.
