It's actually a common misconception. Simply the looks of Asian actors for someone that's not too familiar with them. I usually have this issue with Takashi Miike's yakuza films, where you have too many characters. I end up confusing one with another.
You know that stereotype, "all Asian people look the same". Even if it's not true, for the untrained eye, the sister and girlfriend can look pretty similar.
It's called "cross-race effect", and it's a documented condition. It's easier to identify different characteristics in your own race than on different races. As a caucasian, I had problems with The Departed, for example, DiCaprio and Matt Damon being kinda similar. I assume that this problem was more pronounced for Asian people.
Asian cinema understood this problem. And they made more obvious differences in movies that are meant for western markets. Age differences, different hair styles, in order for other races to have an easier time making difference between the actors.
