Sometime it happens, sometime it doesn't. Is there a pattern at all or is it random.
I noticed that the zombie in the 1st Floor Mansion hallway outside the bar it happens almost every time with the Beretta after 4 or 5 shots.

If i remember right , it was something ridiculous like 1 out of every 99 shots was a decapitation, which one, was pure chance, but chances go up with every bullet, so by the 99th shot if you havent gotten one it would be for sure. so if you managed to get 2 in a row, your one lucky mofo, happened to me once, one crimson and a regular came at me. And also some are scripted into the game like the one on the second floor balcony when you first get there.
