For two reasons:
1.) If you live in South America and see a bunch of white guys running around, what are you going assume their nationality is? American, obviously.
2.) "Gringo" is a term that encompasses all white people, not just Americans.

1. There are tons of South Americans (and people around the world) that are white and there are tons of Americans that are not white. The US is not the only country with white people.
2. "Gringo" is a word that has always been associated with americans, not just any white person.
Gringo means foreigner, usually a white person, it does not mean American.
