What does a 'horse face' mean?

Does it mean a "long face" since horses have long faces? Or does it mean having a "wide mouth" since Julia Roberts has an unusually "wide mouth" when she smiles/laughs?

I think it means a long face. There's that old joke that goes "A horse walks into a bar, he sits down, and the bartender asks him- Why the long face?".
