Probably he quit for a while, and that made him gain weight (understandable for a recovering drug addict).

-) they had to cut his nose off
-) he got forced to get into a program (after all, I guess he had to check into a hospital to get his nose removed)
-) by the way he's been in jail (they mention it) so he might have been forced to quit there
-) maybe, when we see him in the movie, he has just recently restarted with his old habits
