I have a question about the end of this movie

How did the guy's spirit at the end of the movie end up trapped in hell if we never saw him die?I don't get this.

After opeing the box the Cenobites come for him and if you don't blink you'll see Pinhead kill him when he opens the door and Pinhead is standing in front of him.

Then he wakes up, and from that point onwards it's all his own personal Hell.
