So when he opens the box in the bathroom and the lights turn off, this is supposed to be the moment when he goes to hell, as I have understood it. However, prior to this moment in the movie he found that finger in the candle, which tells me that he was already in hell and what we are watching is him repeating the events for another cycle. Is this the case or not?

Depends on your view. Note, the finger in the candle was at the scene of a guy who wanted the puzzle box. in the novella Frank had a plate of doves heads when he opened the Box. So, by that account, the finger in the candle at the scene in the "real world" can easily be simply that.. an unbexplained aspect of a bizarre case... but one which is incorporated into Joseph's Hell as it was part of the case he was working on. Note, when he 'wakes up' all other times it is in the bathroom.
