When a character asks a question, the other person says, "I'm sorry", so the character asks the SAME QUESTION, and somehow repeating the exact question clarifies it.

Its very annoying.

I took it that the "I'm sorry" meant that they hadn't heard or not understood the question, so it needed to be asked again.

Agreed. That did become annoying after awhile.

I don't think it was through not understanding the question, but more like an incredulous "what the hell are you are thinking!?" kind of patronising retort.
