How did he sneak up on Sadako?

When Sadako is standing by the well and the Dr. comes and whacks her on the head and pushes her down I found that very hard to believe. She has very powerful psychic abilities (she can kill people just by wishing it) yet she can't sense he was sneaking up behind her? Lame!

Ringu 0 explains that. Sadako's "father" injected her some drug that weakened her. Then she ran outside and at the moment when she reached the well, she was already suffocting and couldn't stand straiht without leaning on the well's rim.
