How come Malcolm could realise he was dead, but none of the other ghosts could?

Because Cole helped him with that. At that point Malcolm was able to let go and move and, so he was ready to accept the truth.

Now that Cole knows how to deal with other ghosts, I'm sure they'll be able to face reality as well.

That sick girl that came to him as a ghost passed onto the afterlife. After Malcom encourages him to help the ghosts, he will help more ghosts pass into the afterlife.

I think you meant to write COULD NOT realize he was dead.
